Use Adobe Acrobat or Adobe Reader to open the form from your computer. Viewing and completing a form in a web browser may result in information not saving or printing correctly. Forms requiring a signature should be completed with your information, printed, signed, dated and submitted to the Office of the Registrar either in person, by email as PDF or JPEG, or by mail.

Correspondence related to your request will be sent to your CNU email address. If you do not have access to your CNU email address, the personal email indicated on the form should match your personal email address listed in your CNU student record.
